What is Emtricitabine / Tenofovir (Truvada, PrEP)?
Truvada is an FDA-approved prescription medication used for treating HIV infection. It is also used to help lower the risk of HIV infection in people who have an increased exposure or risk of contracting the virus. The HIV virus is sexually transmitted and can lead to AIDS (also known as acquired immune deficiency syndrome). The use of this treatment before a person is exposed to HIV is called pre-exposure prophylaxis (PrEP).

Truvada, PrEP (pre-exposure prophylaxis) is a pill taken once a day to lower the chance of contracting HIV from sharing needles or from sex. You can take it if you have had an HIV test and are negative. PrEP is 99% effective in reducing the risk of contracting HIV when taken daily.

How do I know if PrEP is for me?

PrEP is a good option for anyone who is sexually active in a relationship with a person living with HIV, someone who is not using a condom on a regular basis, and for anyone having anal or vaginal sex. HIV can be transmitted through someone's bodily fluids and lives in the following: semen, vaginal fluids, blood, rectal fluids, human milk, and pre-ejaculate.
PrEP is also beneficial for preventing HIV contraction through IV drugs and needles. Important to note that PrEP is beneficial in preventing the contraction of HIV. Who can use it:?

Prescribed Truvada, PrEP can be used in adults and children who weigh a minimum of 35 kilograms (77 pounds). Prior to starting PrEP, a person must test negative for HIV.

This medication contains emtricitabine and tenofovir. Usually, Emtricitabine / Tenofovir (Truvada, PrEP) can be taken once a day in the form of a tablet by mouth. Truvada works by blocking the enzyme called “reverse transcriptase” which HIV uses to copy its genetic material and reproduce, therefore reducing the risk of HIV by preventing the virus from becoming an infection in the body. Practicing safe sex and the use of protection such as condoms is important as this treatment does not guarantee the prevention of HIV.

Possible side effects can include:

Tiredness, headache, diarrhea, nausea, dizziness, depression, a rash, and difficulty sleeping. These side effects of Truvada may be temporary, however, if they last longer, or become severe, it’s important to talk with your healthcare provider, doctor or pharmacist.

PrEP is generally recommended for:

PrEP is prescribed to HIV-negative adults and adolescents who are at risk for getting HIV through sex or injection drug use, experienced sexual assault, and who may have been exposed to HIV within the last 72 hours.

PrEP should NOT be used by people with any of the conditions listed below:

  • Tested positive with HIV
  • Have a history of pathologic fracture (broken bones)
  • Kidney disease on dialysis or Stage 3-5 kidney disease
  • High risk for osteoporosis
  • Hepatitis B infection

How long do I have to take PrEP before it is highly effective?

PrEP reaches its maximum protection from HIV in:

  • 21 days of daily use in vaginal tissue
  • 7 days of daily use in rectal tissues

There is not sufficient data to confirm the daily timeline effectiveness of insertive anal sex or insertive vaginal sex.

How to use:

Truvada comes in a tablet that can be swallowed. This can be taken with or without food.

How often to use:

PrEP should be taken once daily, if possible, at the same time of day. Daily adherence is important for maintaining the efficiency of PrEP.

How does post-exposure prophylaxis work?

PrEP is a 4-week course of HIV treatment in the form of a tablet that helps prevent someone from contracting the HIV infection.
PrEP prevents the virus from reproducing after a recent exposure therefore, reducing the chance of HIV staying in the body, with Prep with cells infected with HIV die off within a short time frame

Is post-exposure prophylaxis effective? Does it really work?

PrEP is highly successful in preventing HIV transmission. It can reduce the risk of getting HIV from sex by approximately 99% when taken regularly as prescribed. There is less data on how effective PrEP is for people using needles and drugs however it does reduce the risk by at least 74% when taken.

When should I start post-exposure prophylaxis?

For starting PrEP (post-exposure prophylaxis) to be effective, you must start taking it within 72 hours (3 days) after potential exposure to HIV or it will not work. It is a short course of HIV therapy to prevent the virus from taking hold of the cells in your body. It should only be used for emergency situations and is not intended for regular ongoing, or long-term use.

What is pre and post exposure prophylaxis

PrEP (Pre-exposure prophylaxis) is medication taken by individuals who are at high risk of being exposed to the HIV virus through sexual contact or drug use using needles. This medication can reduce the risk of the virus taking hold and spreading throughout the body.
PEP (Post exposure prophylaxis pep) is the use of the antiviral drug for people who are HIV negative after a recent potential high-risk exposure to the virus. This is taken to stop the infection and must be started within 72 hours of exposure and continued for 4 weeks.

When should I not take PrEP?

Do not take PrEP if you: Tested positive for HIV-1 infection or if you do not know your HIV-1 status.

Is post exposure prophylaxis expensive?

A full course of the PrEP medication can cost from $600 + without insurance. Most health Insurance plans will cover the cost.
